This mode is used by EATT (#836), but it's a bit large so I moved it out to a standalone PR.

The behavior should be almost same as basic LE Credit-based Flow Control Mode. The only differences are:

  1. Format of packets used to establish and reconfigure(TODO) connections
  2. Multiple channels can be established in a single request
