time picker AM default fix, card border fix, redirect on submit

Why

fixes after merge, time picker wasnt defaulting AM properly in the backend, card borders were getting clipped, and needed redirect

What

• time picker am and pm fix - it now correctly defaults to am in the backend (was showing am visually but not allowing a submission)
• card border clipping fix
• redirect after submit - success screen for 5 seconds then redirects to blade
• form not found modal - let team member know if form doesnt exist
• uses blade member name instead of discord name

Test Plan

• tested time picker by filling hours/minutes without touching am/pm
• tested redirect countdown shows "Redirecting in 5..." and goes to landing page after submit
• tested form not found shows modal with message to contact team member
