Decide and document hosted pricing tiers

• Tiers: Open/Self-host, Hosted Starter, Hosted Team, Hosted Dedicated.
• Launch with flat monthly tiers + task caps; avoid pure usage billing until task duration and model-cost distribution are known.
• Define what each tier includes (familiars, routing, memory, audit, support, SLA).

Decision issue — output is documented pricing, not code.

Ratified as proposed and merged in #61 — docs/pricing.md is now the documented pricing: Community (free self-host), Hosted Starter $99/mo, Hosted Team $399/mo, Hosted Dedicated from $2,000/mo annual. Flat monthly tiers + daily task caps, BYOM everywhere, and every promised limit mapped to its shipped enforcement mechanism. The four flagged open decisions (dollar ratios, trial policy, blocking-mode placement, OSS discount) carry the proposal's defaults until revisited.
